Clinical benefits
The clinical benefit of bilevel CPAP and PAP therapy for the treatment of OSA is the reduction of apnoeas, hypopnoeas and sleepiness, as well as improved quality of life.
The clinical benefits of bilevel therapy for the treatment of respiratory failure may include; improved overall survival, daytime symptoms, blood gas, health-related quality of life and sleep quality, and a decrease in hospitalisations and dyspnoea.
The clinical benefit of humidification is the reduction of side effects related to positive airway pressure.
Indications
Obstructive lung diseases (e.g. chronic obstructive pulmonary disease), restrictive lung diseases (e.g. parenchymal lung diseases, chest wall diseases, neuromuscular diseases, diseases of central respiratory regulation, obstructive sleep apnoea (OSA) and hypoventilation syndrome (OHS).
Contraindications
Positive airway pressure therapy may be contraindicated in some patients with the following pre-existing conditions:
– severe bullous lung disease
– pneumothorax or pneumomediastinum
– pathologically low blood pressure, particularly if associated with decreased intravascular volume
– dehydration
– cerebrospinal fluid leakage, recent cranial surgery or trauma.
